CCPR Instructions for Submitting an NIH Application

This page lists instructions, format and forms needed for submitting an NIH grant application through CCPR. Click on various links provided to see forms and examples.

NIH PHS 398 Forms

NIH uses a PHS 398 application kit for all grant proposals.  Depending upon the grant type (R01, R03, P1, T32 , etc.) different variations of the forms will be used.   However complete instructions and all the forms are available HERE on the NIH web site.

 

GENERAL FORMAT - NIH general format directions are:

  1. The height of the letters must not be smaller than 10 point; Helvetica or Arial 12-point is the NIH-suggested font

  2. Type density, including characters and spaces, must be no more than 15 characters per inch (cpi). For proportional spacing, the average for any representative section of text must not exceed 15 cpi;

  3. No more than 6 lines of type within a vertical inch;

  4. Margins, in all directions, must be at least ½ inch.

We have found that Arial-11 or Helvetica-11 maximize space without exceeding the requirements above.  Note be sure Acrobat Print settings do not have the box checked for "Shrink oversized pages to page size" as this can decrease font to an unacceptable size.

 

All pages should be numbered consecutively.  CCPR will do this when all documents below are completed. On the Research Plan include a running header with "Name of project" on the left and "PI: Name of PI" on the right.  See below for Templates for the Research Plan.
 

PHS 398 FORMS AND EXAMPLES

CCPR will help with all administrative aspects of the application.  The following lists the support CCPR will provide on each form and information needed from the Principal Investigator (PI) writing the proposal:

FACE PAGE –  CCPR will fill this out.  Requires PI's signature.  Need finalized budget figure and project title before signing.

DESCRIPTIONCCPR or PI can fill this out.  PI Needs to provide abstract and identify key personnel.  PI also needs to note any performance sites other than UCLA.  Follow additional instructions noted on the actual form below.

TABLE OF CONTENTS -  CCPR will fill this out upon completion. PI should submit list of any appendices early so copies can be made in advance of mailing.  Review this form as it shows a good outline of the entire proposal.

DETAILED BUDGET YEAR 1CCPR will fill this out based on budget approved by PI.   Not required for modular applications (under 250K direct/yr).

BUDGET ENTIRE PERIOD (Up to 5 years Max.)– CCPR will fill this out based on budget approved by PI.  Not required for modular applications.

MODULAR BUDGET    CCPR or PI can fill this out. Input is needed from the PI after the budget is finalized.  Uses $25k direct cost increments. Format for the modular budget is provided below.  Completed NIH Example is here.   NIH generally wants funding kept at same level all years.  They allow variance with detailed explanation.  This is the only budget detail provided for modular applications.

NIH BIOs - NIH uses specific format. CCPR can assemble NIH Bio from CV but will need to be reviewed by PI.  Old Bios can be updated by CCPR as well.

RESOURCES PAGE    CCPR can do this with input on any specific resources that are important to the project.  A typical format is shown in the example.  Let CCPR know if it is OK to use this format or if specific modifications are necessary.

PERSONAL INFO.    CCPR or PI can fill this out.  NIH wants the following personal information submitted for PIs.  This is voluntary and is not given to reviewers.

APPENDICES    Try to decide on the inclusion of any appendices early so copies can be obtained.  Include list and reference in research plan accordingly.

OTHER SUPPORT    Is NOT required up front but is requested upon successful review.

HUMAN SUBJECTS TRAINING CERTIFICATION.    Required upon successful review and recommendation for funding. Expires every 2 years.  Online training sites is available from NIH and UCLA.  See CCPR's Human Subjects web page for details.

Other Proposal Forms Needed

 UCLA forms needed for any proposal: 

  1. Goldenrod -  CCPR will fill out and route for approvals.  Needs PI signature.  Needs final budget total and project title before completion.  (PDF, CCPR Example)

  2. Economic Interests and Financial Disclosure  -  CCPR will fill out.  Needs PI signature. (NIH or NSF use 740, Other clients use 730, No Example)

  3. IRB review request or claim for exemption.  CCPR can complete exemption form (HS-7) but requires PI input on data sources and usage.  Full IRB review (HS-1) requires detailed PI effort but CCPR can help. See CCPR's Human Subjects web page for details.

Proposal Scheduling

Regarding timing, the budget is critical to completing many forms and should be finalized 1-2 weeks before submittal.  CCPR grant proposals can be signed by ISSR up to $500,000 direct cost.    ISSR should be given 1-2 days for review and approval of complete package.  When UCLA Contracts must sign, they require 2-4 days for review, especially during NIH cyclical due dates (10/1, 2/1, 6/1).  To avoid emergencies, non-budget forms and the research plan should be given to CCPR 1-2 days prior to being submitted to either ISSR or UCLA Contracts.  This means ideally the PI should aim to complete their portion of the proposal 4-6 days prior to the due date.
